Amiral Pkg-4 (HDEC) developed QR Code HSE Observation Initiative to provide an instant, easy, and transparent HSE reporting system for all personnel in the Package 4 project. This initiative is designed to enhance our proactive safety reporting culture, ensuring continuous improvement in health and safety, and maintaining a strong safety record for the project.

How to Use the QR Code:

  1. Scan the QR Code:
    • Use your mobile phone to scan the QR code.
  2. Access the Safety Observation Form:
    • Scanning the QR code will give you direct access to the safety observation form.
  3. Submit Your Observation:
    • Write down your observations on the form.
    • You can also attach any relevant photos to provide a clearer understanding of the issue.
    • Once you have completed the form, submit it.
  • After you submit your observation, the HSE office will review it.
  • The observation will then be forwarded to the respective subcontractors or departments for corrective action and close-out.
  • These observations will be tracked through an observation register which shows the open and close items.
  • Details of total observations including number of open and close items will be included in the weekly and monthly HSE reports.
